usually, i double-space the diversity statement at 11 pt font. I wouldn't go anything below 11 pt font.
As for the addendum, I generally try to keep it consistent so I also use double space. -

I always used the 7sage proctor app for PTs. Also, it helped on the test day to think of the exam as PT 86 instead of the actual thing.
