The label can be added to the fMRI diagram because we can reasonably assume that a place conducting these scans will HAVE to have names or "labels" to associate the info to a patient. Like J.Y. said, just because they CAN recreate the face and then track the person down (using facial recognition etc.) doesn't mean it would be easier than labeling the document with a way to track the patient like used for genetic profiles.
As for answer choice C - it is out of left field to me. I think the LSAT authors want you to make a quick assumption about privacy being in the stimulus so they put it as an answer choice but if you refer to the stimulus and take the time to ask yourself if the claim is backed up by the stimulus (like in blind review) we would find that it is not supported at all. I have made similar mistakes and have to remind myself the real world doesn't matter ONLY what you can see or reasonably assume in the stimulus.
While I understand your rationale, it seems that "where" is being used for grammar purposes and not as an indicator.
This is the type of question the instructions are warning us about "Always take a step back and consult your intuition. What is this sentence saying? Can I frame the relationships with conditional logic in order to achieve clarity?"
Even though your logic works(I also originally thought this), it is not necessary for clarity.
I believe the parentheses are used to "group" the conjuncts together to show they are linked but still independent to each other in the fact that not both need to be true. It is also used with the negation "/" so it is being used to show that both /N and /N will be negated.
I find that writing the first letters of words or phrases as my symbols helps me refer to the stimulus to better understand the translation. My symbols were 9-5LD -> 15C and AOLD -> 10C (AO = all other calls.) Even if it "takes more time" or I can kick an idea to the domain, I can understand the translation and get the answers correct.
