I have been having the same issues as well. One of the things that I learned is I needed to calm down (easier said then done I know), but if I feel stuck at a question I just will sit there and breathe and reread it until I understand it. I will also flag it so I know what to look for when I am done with the blind review and want a summary of the question. But ignoring the clock and understanding that I just need to breath and focus on the question has really helped, and my score has gone up on every practice test since.

Also when I was struggling another tool that helped, was the LSAT demon youtube/podcast. I would listen to them while tanning or relaxing because they helped explain some areas that I (still now) struggle with.
Don't be discouraged I took the june LSAT and got a 138, I am now taking the nov one and just have been studying nonstop. My score has increased somewhat but I know that your personal statement and other things also will carry significant weight as well.
