Your explanation for D is partially wrong. The passage tells us that "in each case it turns out that the rash occurred on the side of the face which a telephone was held". From this information we can conclude that the dermatologist knows that his patients were using telephones. Thus, his patients ARE the same people in an industrialized world who use telephones.

hmmm after re-reading your comment it makes sense... perhaps it doesn't need to be an industrialized world. But we know for sure they are using a telephone.
