While I don't think it's necessary, I don't think it would at all hurt your application to include a short and sweet explanation of the circumstances. Obviously your GPA is great and you fulfilled the requirements of your degree, so it's not like you were withdrawing from essential courses. Not including an addendum would probably be fine but I know I'd probably stress over whether Admission Committees were confused by the W's, especially since they were concentrated to one academic year. If I had 3 or more in a year, I'd probably write one but obviously up to you

I guess it depends on the schools you want to apply to, but with a 3.95+ I wouldn't bother writing an addendum.
