    @akistotle concerning necessary documentation, I was confused about how to prove I have the condition I need accommodations for. I called LSAC and the women on the phone barley had an answer for me. My doctor just provided a consultation note that states my condition under "assessment." Is this appropriate? (My doctor provided her personal statement as well) I didn't know if I needed to provide blood work or not, and when I asked the lady on the phone she said that seemed unnecessary.

    LSAC asks for proof of diagnosis so you need something that states you have your condition. I provided my test results.

    Something that says X has been diagnosed with X should be sufficient. You should be fine.
