How do you distinguish between answer choice A and answer choice D? While I understand why A is correct, why is D incorrect? If almost all citations result in fines, shouldn't "nearly all" of the landowners at 48+ hours be fined?

It could be that the city is very punctual cleaning all the sidewalks between the 24-48 hrs timeframe. Thus, although the landowners "do not clear their sidewalks within 48 hours", the city does. And they will not be fined, just charged for the service.
Additionally:
"...will receive citations, which always result in fines unless the landowners can demonstrate extenuating circumstances"
D is incorrect because it could be the case that all of the landowners who were unable to clear the snow were on vacation during that period and were able to demonstrate that.
