Hey guys,

I had a small hiccup with this redo and it cost me a point. I thought that the antibiotic has the option of one, two or three in, not 2 max since the first rule is conditional. If I put three in antibiotic, one in dietary regimen, I figured I could put one in phys therapy. I'm not sure what exactly I can look for to come to the conclusion that there is 2 max.

Please help.

The first rule is just a "fake conditional". Their statement that "if two are in the third has to be out" is just a fancy, confusing way of saying "a maximum of two are in".
If you were to have three in, that would trigger the rule, because three being in would definitely mean two are in, so the third would automatically be out, resulting in a contradiction.
If you were to interpret the rule to mean "if exactly two are in, then the third is out", then it wouldn't be much of a rule - of course the third is out if exactly two are in. LSAT writers would never have a rule that is both unclear (leaving out an "exactly" when they mean exactly) and useless (stating the obvious).
