Thanks everyone. Pacifico, when I went on the site it didn't work, tried emailing the emailaddress they have on the site, but did not receive a response. After what bishoplaw mentioned, I'm not sure it is worth it.
In most cases you email it to them, other schools ... as an attachment if you email it. Some schools will have ... continued interest, such as which emailaddress you should send it to ... can send it to the emailaddress you got the hold or ...

it's typically done by email. You go on LSAC and ... info for your recommender (name, email, address, etc.) and then you choose ... send them the request by email. Your recommender then will write ... given to them by the email you send to them.

I think it just redirects you to email their generic admissions emailaddress. It's not a special form or anything. I asked for a merit based waiver and got one (well, got several as I was confused like you and asked in multiple places :-P)
