Depends on the school. Generally phone call, email, IRL email, status website, or sometimes you’ll know from being able to create an account on the admitted students website

I don't think there's really any hard cutoffs. Depends on the applicant's GPA, if they have a URM status, maybe their background too. It just really depends..
While "soft factors" such as your master's degree do matter, your LSAT score and undergraduate GPA are "hard factors," which are the most important factors in the admissions process.
