What do the scores on top and the percents at the bottom of answer choices reflect? I tried to search for a previous explanation but could not find it.

Those scores are the average PrepTest scores of people who chose that answer. So, for 35.4.22 where it says 159 for D, that means of everyone who took PrepTest 35, we look at the subset of those people who chose D for that question and we average their PrepTest scores.
The percentages are the average percentages of people who chose that answer. So, for 35.4.22 where it says 8% for D, that means of everyone who took PrepTest 35, 8% of those persons chose D. See the screenshot below:
