Yes you do your checkin ou can the proctor would have to see them when I used them once they weren't for me but double check the permitted items allowed rather be safe thansorry
The place I took the test at provided noise canceling headphones. Initially I did not use them.. but when it became obvious other testers were going to be coming and going, I put them all.
Keep that in mind as well. Most of these testing centers are not just offering the LSAT. In the time it took my to take my test a dozen people came and went.

You would have to call the LSAC for this one, I believe they have certain things as to what is or is not permitted during testing on their website

Yes. You can bring soft, foam earplugs that are not connected by a wire into the testing room.
