It depends on their policy, some online providers are not able to fill out accommodation forms, so you would have to ask first. I suggest its best to see your GP in person with a diagnosis or summary visit from your mental health provider along with the form. In the end, as long as the person has a license and is a professional that fills out the form its all that matters.
