This is really not a specific enough question to receive a helpful answer.
There isn’t really such a thing as an "LSAT accommodations provider." If you have a disability that requires accommodations, you need a provider who is qualified to diagnose and treat your specific disability in your geographic area.
To even point you in the right direction, people would need to know (but don’t answer this on a public forum):
Where are you located? Providers are licensed by state.
What are your functional limitations? Having a diagnosis is not the same as having a documented test-taking limitation.
What is your history with accommodations? Have you had an IEP/504 in the past 5 years? Do you currently receive them at college or work?
For high-stakes exams, as well as formal requests for accommodations in school (K-12, university, and graduate school), the burden of proof is extremely high if you don't have a documented history of accommodations. However, if you already have extensive, up-to-date documentation of your disability (such as recent comprehensive evaluations, IEP/504 plans, or a record of approved accommodations on prior standardized tests), you may not need to find a new provider at all. Your existing paperwork could be enough. Sometimes, your primary care provider can manage your treatment and sign off on your limitations, provided they already have your diagnosis on file. But if you are starting from scratch, here is what the process typically looks like:
For mental health, learning, and developmental disabilities: The standard is a comprehensive neuropsychological educational evaluation. If you don't have recent high school or college documentation, this is generally the expectation to secure accommodations in educational settings (K-12, university, graduate school) and on professional exams (including the Bar). Be aware: these run about $2,000 out-of-pocket unless you find a university teaching clinic. This evaluation will specifically detail your functional limitations and exactly how they impact learning and test-taking. Alternatively, you may be able to have your existing provider fill out a provider form, if they’ve been treating you for a while and they know you well.
For physical and medical disabilities: You generally need a diagnosis from a specialist in that field, plus the specific provider forms required by the exam board or academic institution.
For all disabilities, you generally need to clearly describe your functional limitations and provide a strong Statement of Need that directly connects those limitations to the specific accommodations you are asking for. It is a rigorous process, and depending on your limitations, you may need a combination of accommodations rather than just a single adjustment.
Q: What is a functional limitation?
A: A functional limitation is a specific, measurable way your disability impairs a major life activity (like reading, processing speed, or sitting).
Example: Having a diagnosis of "ADHD" or "Anxiety" is not a functional limitation. A functional limitation is, "Due to my ADHD, my working memory and sustained attention are severely impaired, causing me to lose track of complex reading passages after three minutes and requiring frequent breaks to refocus."
Q: What is a Statement of Need?
A: A Statement of Need is a clear explanation that bridges the gap between your diagnosis and your request. It directly connects your documented functional limitations to the specific accommodation you are asking for, proving why the accommodation is necessary to level the playing field.
Example: "Because my working memory and sustained attention are impaired to the point that I lose track of reading passages (functional limitation), I require 50% extended time (accommodation) to allow for the necessary refocusing time to accurately demonstrate my aptitude on the exam."

If you have a psychiatrist therapist or a pcp or someone who medically or psychologically knows about your issues and can attest to them. I just set up an appointment and sent them the application from the LSAC Accomodations page and they walked through it with me like an evaluation. They sent it to me to upload to LSAC and a couple weeks later I was approved I hope that helps!
