Same, I was stuck on figuring out whether or not the speaker considered the harm preventable or not. It could go either way in my brain. One could argue the harm was NOT preventable, because the factory was not aware. And then D would be correct, because it's saying, even if you weren't aware, you are responsible.

But luckily I landed on E.

You say we should automatically recognize that the answer will not be a negative (aka "an art museum should not.."), but what if an option was "An art museum should not collect any works that are not high-quality?" Would that not be an appropriate correct answer? Of course Simpson isn't talking about what an art museum should not do. But does that really mean the response cannot possibly exist as a negative?

After getting this wrong, I had to reflect deeply on how I could actually grow or change to not make mistakes like this again. Like, do I dismantle my entire self-confidence? The same thing that serves me well on other questions? I fear I've learned nothing from this mistake.

@Sunday_Blues13 I know. I get 100% consistently on all the practice ones featured in the videos, then often get 60-80% on the actual drills like this, where there isn't an instructional video to go with it and help break down discovering the answer.
