Can a therapist fill out the qualified professional form? I’m trying to get accommodations for ADHD but my psychiatrist is unable to fill it out. would my therapist be able to fill it out?

Mine was approved within a week and was filled out by my therapist. She is a LCSW. She wasn’t sure if it would be approved or not, but we tried and it was. I would always try early with what you have. They should get back to you within a week or two if it is approved or not!
I'm not sure if a form filled by a therapist will be acceptable for accommodation on LSAT. From what I've read and heard, a psychiatrist should fill it out for it to be acceptable.
