Applying for Accommodations

Hello,
I'm currently working on my application for disability accommodations and had a questions for those of you who went through the process. I was wondering if the documentation from the qualified professionals needs to be submitted by me or by them individually? Your help would be greatly appreciated.

You get them to fill it out but you send it to them along with the other files that are required!
