I split one of my pieces of scratchpaper into 4 quadrants, and treat each one almost like a mirror of the passage. Write down a number for each paragraph and write notes next to it.
No -- you supply your own paper (max of 5 sheets) that you show both sides of before taking the test. The scratchpaper does not have to be in view of the camera but you yourself must stay in the view.

I did not rip my scratchpaper (I didn't use it at all, not sure if that matters or not) and was worried my ID picture would be too blurry, however I received my approved writing sample in 4 days.
As opposed to the normal format test? Or digital vs paper? Either way, you'd just write whatever you need to diagram on paper... in my opinion that shouldn't be more than a few questions. Not quite sure what you're asking though.
