Have you tried reachingout to the school directly to see what the status of your application is? Plan for the worst, but hope for the best. Don't assume you haven't been accepted until you confirm it with the school :)
It depends on when and where you apply. I know some schools that send outadmissions to people with just a few weeks before the start of classes. But generally if you apply early you should get a response early as well.
It depends on the content of each essay, but I think it's always good if you can cut down on what you're asking the admissionsoffice to read. If you cover your diversity factors in your PS, you don't need to write a separate DS.
