Once you are registered with the CAS, law schools obtain your information and based on that they send you feewaivers (correct me if I'm wrong). I hope this helps.

There isn't one specific answer. I know of 2 people who received Columbia waivers. Their scores ranged between 167-171. One of the applicants was a URM, so not sure if that changed the calculus at all...
I don't think they have a specific score range for feewaivers. My score on file isn't impressive, and I received one last month. My friends with a 172+ did not receive one.

Yeah as Alex pointed out, most international students are not eligible for feewaivers.
Having to spend roughly 6% of the GDP per capita of your country on the exam sucks.
