I titled mine, though only because I knew the title early on and it helped me shape the piece conceptually. My PS was on the literary side, so it seemed appropriate. I don't think this is a big deal at all. However if you include a title, make sure to also include it in your word/page count.
Apparently it's a no-no. So I didn't either. Wondering if there are other opinion...
I also didn't. It's not necessary.
I didn't either. Just put your name,the label "personal statement", and your lsac number in the header and you'll be fine.
