So many times when I am reviewing what I got wrong, I look back at the question and I'm like why the hell did I choose that question. Why does this keep happening.
Also, most times I end up choosing the second best answer and how can I fix this.

Most of the time, it's due to missing key words in the stimulus. With my ADHD I often read over certain things or read too fast without really paying attention.
This used to happen a lot to me, but it's gotten better and what I do now is when I'm between 2 answer choices I focus on the wording of both answers. I compare the strength of language of the answer choice to the stimulus and in general try to find a word or so in the answer choice that makes one more likely to be wrong.
