Just disclose by laying out the facts of the situation. I doubt something like that will hurt your application. You don't need copies of the charges though, don't worry about that. Just a short and to the point addendum.

Probably not, seeing as you usually have a word count to your addendum and there is no place to upload medical documentation. If you have a medical condition that requires accommodations in law school, you can show the school your papers when you enroll.
