You will see on the applications it asks for current address, and permanent address, and for how long each is good. They also ask to where do you want mail sent. Which is your state of residency, etc.
If you move, you should also have your mail forwarded in case the school mistakenly sends more mail to the old address

I believe they mail you the refund after you sign up for a real administration. I've forgotten if it comes in after payment or after you take the test but I recall one of their info emails mentioning which it is.
I was told they mail the refund check after the official score has been released. Wasn't given a time frame, but score must come first to ensure no cancellation, and then the refund will be granted.
Did you receive something in the mail from LSAC regarding you not signing your answer sheet ? It looks like until this is fulfilled you won't get your score. During the LSAT you must have forgotten to sign your answer sheet.
