2) Yeah, ETA is definitely employment (ridiculously fun employment, but still). I listed Fulbright under work experience on my resume and plan to list it under employment for LSAC apps.
The two-page format started with PT 66 in the 62-71 book. I was also wondering how some of the older videos got the new format. Guesses: licensees can get them that way from LSAC or they can, and 7Sage did, cut and paste.
They will provide it for you and there is an example on the LSAC website. It is a large print format. If you were approved for your test booklet then you just mark in that only.
